AGREEMENT In consideration of my enrollment in the dog training class conducted by KDTC and in recognition of KDTC's goal of keeping fees at a minimum, I:
  1. Waive any and all claims against KDTC for any damages, including - but not limited to - personal injuries and property damages sustained by me or the dog I bring to class, during any class conducted by KDTC.
  2. Agree to indemnify and hold harmless KDTC, its directors, officers, instructors and staff against any claim asserted against KDTC for all damages, including - but not limited to - personal injuries and property damages sustained by any person if such injury was caused by me or the dog I bring to class.
  3. Agree to ensure that my dog is inoculated against DHL/PARVO and Rabies (if over six months of age) before bringing the dog to the first class.

UNSIGNED forms are not accepted! Class fee $50; reduced fee $25 for dogs adopted from Animal Control or Rescue Organizations if enrolled within 6 months of adoption (you MUST enclose a copy of the adoption contract); fee waived for service dog candidates (CCI, Leader Dog, etc). Pre-registration is advised as our classes often fill several weeks before registration. You will not be contacted unless your selection is full. Make check payable to Kalamazoo Dog Training Club; mail form and fee to:
